No Time to Find Solution: There is simply no time to make project planning the right way. The clients want their products within the deadline despite several change requests. Developers want to develop things but are not ready to sit for project planning meetings. Vague requirements and quick fixes result in incomplete deliverables with half-baked solutions.

Not Stopping to Check for Repeat Mistakes: People are ready to accept that mistakes will happen during project planning. Have you gone back to the previous reports to check if the team has been resolving the same bugs? Inadequate project planning and documentation processes cause duplication of efforts and delayed deliverables. People are okay with the costs incurred to fix those repeating mistakes but will not make time for a final solution.

Solving Problems on the Spot Is More Heroic: People that can solve problems in a jiffy are heroes. They could have easily avoided that by conducting regular project planning meetings. Several project managers have risen to fame as problem-solvers because they were spontaneous in giving solutions. Many believe that the project planning phase is a waste of time because you have to change the requirements again later. They forget to realize that they could have prepared a better solution rather than temporary fixes for the issues.
