Your Issue Log Should Consist of These 3 Essential Items

Projects must have an issue log where you can document all the issues. There are factors that you cannot avoid or restrict during the project lifecycle. In this article at PM Tips, discover the 3 essentials items you should include in your issue log.

Teams make assumptions and register them in the risk register. When these risks occur for real, you document them in the issue log. There might be some issues that never occurred to anyone and hit the project directly. To be on the safe side, include the following essential items in your issue log:

Have a Detailed Issue Description: Do not write only the title of the issue and leave it. Document the details in the issue log. The report should describe the incident and its causes, impacts, and intensity. You will not discover the impact of the issue right away, though. Conduct detailed analysis and identify the actual issues over the course of a few hours or days. Employ your intuition to figure out how this could affect the project and the company. Can you handle the issue? What skills do you need for issue mitigation? Do you need external help? Will the allotted budget be enough to cover the incurred costs? Include all this in the issue log.

Mention the Issue Owner: Every issue must have an owner. Mention the name of the issue owners in the log along with their roles. Ensure the owners have a team of people ready to immediately address the problem. The owner can be a subject matter expert instead of a project manager. Project managers should not be the contact person for all the issues. They can be distributed among other roles in the team. However, project managers must handle issues that need senior management involvement or high-end handling.

Work on the Action Plan: Once done describing the issues and appointing an owner, describe the action plan in the issue log. The plan should be based on the issues, so assess them well and work out the best possible moves with the team. If there is no space in the issue log, leave a reference to where the team can find the action plan details. For example, add a gist in the issue log and write in details in the project schedule.
